Hi this is Dr. Diane Gay Hart and this is my talk on the case concept based on chapter 2 of my book Mastering Competencies in Family Medicine so in this talk I’m going to walk you through how to put together the case concept, and I’ll be right with you from the beginning, in that case concepts take time, and They work, and they think, and so on. Maybe they’re a little on the difficult side, and to be honest, it takes you 10 to 20 hours to put your first case concept together, and I know it’s like a lot of time, but you’ll find out, too, and it’s the most you’ll ever fill. What I really believe is a practically useful form and why that is is sort of walking you through the main conceptual and assessment areas from each of the majors. The theories covered in this book therefore mean that it takes you through all the main assessment methods for the concept of working with individuals. Mainstream theories and couples and families from the field of family therapy, so it’s a bit of work and a bit of an intellectual exercise, and yes, but I would say it’s one of the most useful things you can do because it allows you to think through your problems in a more holistic and theoretically complete way, and I I will tell you when I engage in my own clinical practice, what I basically do is conceptualize the case. In this chapter I’ve explained to all my clients to help me stop, so I hope it’s a useful tool for you many years down the road when you’re stuck asking why people aren’t. It provides a template for developing an individualized treatment plan. Importantly, case shapes are flexible. They describe all information collected at any given time and are updated to reflect new information as it becomes available. The models of anxiety and depression presented in Chapter 8 are well-known, that is, they describe the general “laws” of how these problems arise. The purpose of creating a case is to create a conventional model that applies only to a particular case. Individuals (1989) identifies several other roles in case formation. First, by ensuring that the therapist understands and communicates the problems that arise in the client’s personal situation, it conveys hope that these problems can be resolved. Second, it guides the development of a treatment plan that deliberately targets the underlying causes and conditions of problems, increasing the likelihood of an adequate response to treatment. Third, it can overcome barriers that may impair treatment progress or communication. The treatment plan can then be adjusted to proactively address potential obstacles, making it less likely that the obstacle will prevent progress. Several different methods have been devised to guide the generation of case shapes (Boschen and Oei 2008; Macneil, Hasty, Conus, and Berk 2012; Persons 1989). Surprisingly, however, very little research has been conducted on the clinical utility and accuracy of case forms and subsequent treatment plans, particularly in child and adolescent populations. While inter-rater reliability of case formulations has been found to be adequate (Kuyken, Fothergill, Musa and Chadwick 2005), the quality of case formulations improves with therapist experience and training (Eells et al. 2011). When the clinical utility of case planning used in a community setting for adults with anxiety and depression was investigated, case-planned treatment resulted in statistically and clinically significant change (Peoples, Roberts, Zalecki, & Brechwald 2006), compared with one found in randomized controlled trials of clinically supported treatments. A direct comparison of case-form therapy and routine therapy has shown some superiority of the former (Persons and Tompkins 1997). Cognitive Behavioral Therapy (CBT) for Child Anxiety CBT interventions have been shown to be most effective when tailored to the individual’s specific anxiety symptoms (Eisen and Silverman 1993, 1998), and for the majority of people, effectiveness is established long-term. Individuals following a CBT program for adolescent depression included the use of a case formulation (Treatment Team for Adolescent Depression [TADS] 2009). The case-form approach has also been shown to improve some aspects of the therapeutic alliance (Chadwick, Williams and McKenzie 2003); and acceptance of the client’s treatment philosophy—an aspect of case design that is related to better treatment outcome (e.g., Addis and Jacobson 2000).
