High-intensity interval training (HIIT) has been shown to improve prognosis and health outcomes in patients with cardiovascular disease. This study aimed to determine and summarize the effects of HIIT in heart failure (HF) patients, heart transplant (HTx) recipients, and HF patients before and after HTx. This systematic review is based on the Priority Reporting for Systematic Trials and Meta-Analyses (PRISMA) guidelines. A systematic search was conducted using PubMed, PubMed Central, Cochrane, Google Scholar, and ScienceDirect databases within the last five years from September 15, 2021. Studies were selected based on the following predefined criteria: randomized controlled trials (RCTs) in English, observational studies, systematic reviews and meta-analyses, including HF patients and HTX patients and evaluating the effects of HIIT. Relevant data is exported to a predefined template.

Therefore, a quality assessment was performed using commonly used assessment tools in each study. The initial search yielded 551 studies. Nine studies were included in the final selection – four RCTs, one cohort, one quasi-experimental study, two systematic reviews with meta-analyses and one narrative review. Survival rates for HF or post-HTX have increased over time, with one-year survival rates of 80–90% and 91%, respectively [4–6] . Despite this, these patients have a subnormal quality of life (QoL) compared to the average person, and their prognosis may worsen over time, highlighting the need for evidence-based improved therapies.

All references were collated and alphabetized using Microsoft Excel 2021 to avoid duplication. Articles were first screened based on title and abstract, excluding irrelevant studies. After review, full-text articles were reviewed. Study protocols were excluded from this review due to lack of relevant analyses.
